Evaluation Purpose and Guidelines

St. Luke's Episcopal Health Charities (SLEHC) requires all grant recipients to measure their success in attaining program goals. The purpose in asking for an evaluation plan in the application phase is to encourage potential grantees to incorporate evaluation into their program plans as early as possible.  By defining the evaluation criteria early, organizations will be able to identify additional support that may be needed.  The evaluation plan also sets the report criteria for the required interim and final reports. 

Evaluation Plan - Report Components:

  • outcomes statement (who or what is expected to change as a result of this grant) 

  • the goal(s) approved from the original grant application,

  •  a description of the progress towards the achievement of those goals,

  •  an explanation of any variances from the goals or expected progress,

  •  a financial update (see Financial Report (Budget) example), and

  •  an operation’s profile (see the Operation Profile Tool).     

We encourage each applicant to use SLEHC’s Evaluation Tools to assist in defining and tracking measurable goals and to report and track the program's budget.

Evaluation Reporting Requirements:

All grantees are required to submit an interim and final report using SLEHC's standard report format. In preparation for these reports, grantees are encouraged to attend an Evaluation Workshop to review the standard evaluation components as well as to network with other grantees.

Report Schedules and Components:

  • An interim report on the progress, financial update, and organization profile statistics are due in the offices of St. Luke's Episcopal Health Charities, within 6 months from date of your grant award. 
  • A final report on the project's progress, financial update and operations profile statistics are required by the end of the annual grant period or with the renewal application, whichever comes first.
